An independent panel's report on the Benghazi consulate attacks cites management failures at senior levels in Washington that resulted in "grossly inadequate" security. NBC's Andrea Mitchell reports.
Updated at 10:50 p.m. ET: An independent panel's sharply critical report on the Sept. 11 attacks on the Benghazi consulate blames "systemic failures and leadership and management deficiencies within two bureaus of the State Department" for the post's inability to defend itself.
The report details the events that unfolded on Sept. 11 in Benghazi, Libya, when the Special Mission post was overrun by militants who used rocket-propelled grenades, mortars and machine-gun fire, according to the 39-page report. Ambassador Christopher Stevens and three other Americans were killed.
The report's findings fall largely into two categories: staffing and the physical security of the Benghazi post. Staff was, according to the report, talented but relatively inexperienced. Personnel there spent about 40 days on assignment, resulting in "diminished institutional knowledge."
In a statement Tuesday, U.S. Secretary of State Hillary Clinton said she accepts the report's recommendations. The independent review board was formed at her request and was chaired by former Joint Chiefs Chairman Mike Mullen and former Ambassador Tom Pickering.
In addition to staffing issues, the report says that Ambassador Stevens "made the decision to travel to Benghazi independently of Washington, per standard practice."
The independent report commissioned by Secretary of State Hillary Clinton investigating the attack on the U.S. Embassy in Benghazi faulted the State Department for "systematic failures" and "grossly inadequate" security to deal with the attack. NBC's Andrea Mitchell reports.
Further, the report says, Embassy's country team was not fully aware of Stevens' movements off the compound.
Stevens' "status as the leading U.S. government advocate on Libya policy, and his expertise on Benghazi in particular, caused Washington to give unusual deference to his judgments."
Stevens arrived in Benghazi on a Greek cargo ship on April 5, 2011, according to the report. American embassy personnel had been evacuated months before, in February. At the time, Stevens was a special envoy to the Libyan Transitional National Council.
Stevens was the report says, "extremely effective" and admired by Libyans. He "personified the U.S. government commitment to a free and democratic Libya."
Against a backdrop of mounting political violence, Benghazi became less secure, the report points out in the form of a timeline. The timeline begins with an armed robbery that took place at the British School on March 18 and ends with a small bomb thrown at an Egyptian diplomat's car on Aug. 20.
Stevens arrived in Benghazi on Sept. 10, accompanied by two temporary duty officers. Security staffing at the post on the day of the attack was inadequate and did not meet security standards, according to the report.
The report states strongly that Congress must meet budgetary challenges to "provide necessary resources to the State Department." Specifically, Congress should restore a security program to $2.2 billion by 2012.
Managers, the report says, have become conditioned to tightening the purse strings.
That said, the report continues, the Embassy in Tripoli did not advocate enough for increased security at the Benghazi post. Some security upgrades had been made, although they were not sufficient, including safety grills on windows, concrete jersey barriers and some locally-manufactured steel doors.
As a result, the report says on its first page, "systemic failures and management deficiencies" rendered the Special Mission post in Benghazi "grossly inadequate to deal with the attack."
And although it found that "certain senior State Department officials within two bureaus demonstrated a lack of proactive leadership" when Benghazi asked for more protection, the report said that no employees breached their duty.
The report, although critical, addresses the difficulties faced. Terrorist and hostile actors posing threats to American security are "growing" and "diffuse," it says. Resorting to a "total fortress and stay-at-home approach to U.S. diplomacy" would be unacceptable.
Recommendations
The report recommends that the U.S. should strengthen security in high-risk posts beyond what is provided by host governments and lean on outside experts to regularly assess security at the posts. The State Department should also reorganize its Bureau of Diplomatic Security and appoint an official charged with overseeing high threat posts.
The State Department should also boost Marine security and hire more diplomatic security personnel at high-risk posts, the report said.
The State Department should also improve language abilities, particularly Arabic, among employees, the report says.
Testifying
Mullen and Pickering are scheduled to brief congressional committees on the classified version of the report on Wednesday. Hearings on the report are scheduled in the Senate and the House on Thursday.
Clinton was supposed to testify in hearings on the report on Thursday but she remains at home recovering from a bout with the flu that resulted in her fainting and suffering a concussion.
Her two deputies, William Burns and Thomas Nides, will testify in her place.
Debate over the attacks polarized Republican and Democratic lawmakers, as Republicans questioned whether the consulate had adequate security and whether the State Department had responded to requests for more protection.
At the same time, U.S. spy agencies produced conflicting reports on who was behind them, U.S. officials have said. Most said extremists with possible al-Qaida ties were involved. But a few reports, which the Obama administration emphasized in early public statements, said the attacks could have been spontaneous protests against an anti-Muslim video made in the U.S.
United Nations Ambassador Susan Rice faced intense criticism from Republican lawmakers when she made comments indicating that the attacks were a spontaneous response to a low-budget movie made in the U.S. that maligned the Islamic Prophet Mohammed. She later said she had not meant to be misleading but was relaying intelligence that she had been provided.
Rice dropped out of the running for secretary of state, citing the "very politicized confirmation process."

As far as the WTC attack goes, certainly the intelligence agencies handled that poorly. If we had the coordination back then that we have today it may have gone differently. But you know what? We now have the intelligence coordination today and Pres Obama's administration dropped the ball and 4 lives. They did nothing with the multitude of requests for additional security in the months leading up to the attack, even though the Benghazi compound had been attacked several times over the previous year, the last attack just a few months before the fatal one. Actually, most of our intel on Iraq came from the Clinton Administration, in fact, they went before Congress and said Iraq was a threat, so it wasn't a lie by Bush. Even Clinton admitted that he knew they had weapons back in 2003. A couple of Saddam's top advisers even admitted to having the weapons and shipping them to Syria before we entered Iraq, which is in "Saddam's Secrets", a book written by Georges Sada, Saddam's top Military advisor and General. Yes, we can say we had a failure on September 11, 2012 in Benghazi, Libya. We didn't get it right. That's a fact. Hard to dispute. We screwed up.
It is worth remembering that the day of this attack, which came without warning, it was also September 11 everyplace in the entire world. It was September 11 at every one of the 294 physical embassies, consulates, and diplomatic missions of the United States in every individual location across the world; 27 of those missions located in the Middle East and North Africa region, which is more than any other nation on Earth. Several of those other missions had been actively threatened by demonstrations over the prior 7-day period. Individuals made decisions about where to place limited resources that day, but we didn't get it right in Benghazi, .... and four of our own died there.
It is also worth remembering that on September 11, 2001 we didn't get it right here in America. That time we had eight months of warning, and renewed warnings just weeks before the attacks. Individuals chose not to heed the warnings .... and 2,996 of our own died here.
Perhaps it would be more worthy of us, as Americans, to remember that sometimes we will be less than perfect. It has happened before, and it is going to happen again. We will screw up. We will not get it right. We will not be perfect. And some of our own will die.
One thing however which might help us to reduce the number of future failures, and deaths of our own, is to put as much of our collective energies into finding actual solutions and improving our processes when we have failures ( based upon the difficult lessons learned ) instead of wasting so much of our energies and limited resources on protracted partisan shouting, finger pointing and political demagoguery.
The current tone of vitriol which has been continuous now for more than 3-months, is diverting attentions every single day which need to be focused on the job at hand; the world doesn't just stop being a dangerous place while we are all screaming and pointing fingers at each other.
How much of our limited human and financial resources, and for how long, do we continue to divert and redirect to task of explaining, and defending what has already happened, .... rather than keeping those resources focused on preventing what might happen tomorrow?
Perhaps we all need to dial-down the rhetoric, and grow up?

Also, if I'm not mistaken, and budget changes would not have affected the Benghazi mission when the attack took place because budget changes passed by Obama and the Democratic Congress were for the NEXT fiscal year. The attack took place under the previous year's fiscal budget.

I find that distressing. How long did it take us to figure out what happened ? What was the initial response - a handful of people who couldn't commander any heavy weapons ? Todays technology provides the ability to maintain diplomatic relations with other countries without having to have continuous physical presence of military and intelligence at posts on foreign soil in the name of diplomacy. Until the American government is willing to allow other countries to maintain their sovereignty - just as we wish to remain sovereign and not be dictated to by others - forcing our will on every country and buying allies will only continue to prove it does not serve us. We need to work with other countries to promote peace and stand with them against tyranny. But America has long been both a strong ally and a loose cannon in the name of freedom, when we apply the name of freedom with an attitude of 'our way or the highway'. Our way falls short when our efforts are not genuine and are coersive and deceitful. We should be prepared to protect this country, stop trying to run the world and effect change through being an example of cooperation, constancy, humanitarianism and strength - something we need to work on beginning right here at home. Cynicism in our own country against our own government should keep us plenty busy right now. And how about bringing all those troops home? Sounds to me like there were failures all through the different channels. Let's see, the ambassador doesn't follow protocols, mid-level managers in the State Dept turned down requests from the field, inexperienced security teams on the ground and funding cuts by Congress. I would add that the Whitehouse leadership should have done security reviews for the whole region and our intelligence should have been better. But that requires money to perform any action. This was a government failure caused by both political parties.

And yet with 3000 dead Americans on our own soil the Bush Administration was given an 8 year reign as well, even though they hadn't even finished a single year in office. There aren't enough American troops to prevent anything from happening everywhere on the planet and had there been a larger force in Benghazi who knows if there wouldn't just have ended up being even more causualties. We don't know the size of the force that attacked and a consular building in a backwater town wouldn't be too difficult to overrun, it wasn't a fortress. The death of these people is regrettable, and all the screaming won't bring them back. Just hope that lessons are learned and applied in the future to minimize the potential of a repeat.

The point was that we were told that Jeep was moving American jobs to China to produce vehicles that would be sold here. Not that Jeep was expanding production in China for the Chinese market. Actually, Jeep is opening up in China for the Chinese markets, because it is too expensive to build them here and ship them over. It would still cost too much to build them there and then ship them here. Apparently, it is cheapest to build them in the continent you are selling them. Just FYI.
@Brent they were actually building the Jeeps to be shipped to China here. That's the point. That manufacturing is going to be done in China now, not here. However, Jeep is ramping up production here as well, so hopefully no jobs will be lost. It'd be better for our economy if they were doing *all* their production here, but, unfortunately, sending jobs overseas makes sense sometimes. Sucks for us tho.

We have enough Military that could have provided the security that was needed to save these lives. What a shame and the president stated that he was responsible. Poor leadership at the Department and at the highest levels.

If the school needs security against people with guns, then maybe the NRA should foot the bill for it. More guns is not the answer. More guns will cost more lives, not save them. Let's share a scenario. Pick a Starbuck's location, imagine it being close to fullcapacity, when a guy walks in and attempts to use a gun to rob the place (meaning the establishment and the patrons). Now, there also happen to be 2 patrons (not sharing the same table) that are "carrying" and they open fire on the robber, so a shoot out begins. Now, we get a cop who heard the gunfire and has run over to investigate. Who does he shoot first? Does he shout "POLICE" and hope they stop shooting? Does he just sit there and call and wait for back up? In the meantime, how many bystanders die due to being in the wrong place at the wrong time? A society of vigilantes is not going to make us safer. Too many misperceptions can be had.

I think Rice should resign. I think that the country needs a good explanation as to why the government waffled about this for two weeks after it happened, and changed their story multiple times. To me it was expedient political lying during the election season, nothing more, nothing less. But nothing will happen. No one will be held accountable.

You are also ignoring the time line, what the government said, and pretty much all the facts that were brought out by several different news sources, not just Fox. You can say that your fantasy is reality all you want, but it doesn't change what really happened. All you are doing is maintaining the big lie. Shayneedward: You ignore the fact that Libya wouldn't allow marine guards at Benghazi only the Embassy itself. That the Libyan government provided security along with some private contractors. You also ignore the fact that the request made that morning was never sent and were requests made to the Libyan government not ours. Stevens went to Benghazi knowing it was the 9-11 anniversary and knowing marines couldn't accompany him there.

I can go all the way back to Reagan, being asked for help from Lebanon and yet none was provided and many more than 4 people died, because of a terrorist attack. Maybe what you don't understand is there are protocols for an embassy in a foreign country. I guess the fact that the host country (Libya) didn't have very good troops for security as they fled at the first sign of trouble. Some marines had been flown to Benghazi, but couldn't get out of the airport due to local officials holding them up. There are plenty of instances where admininstrations were "warned" that some sort of attack was eminent, but rarely has anything been done . Also realize that Amb. Rice was given the unclassified talking points to run with, eventhough she had read the classified report. However, she was not authorized to give out classified information. So, you and your ilk seem to think she should have broken the law just to keep you informed? Not very realistic. Did you have this kind of cioniption fit over the original 9-11, when we had plenty of warnings, that were ignored, and tehn the other Ms. Rice, sadi "who would have thought they were going to use planes?", when there was clear evidence available,saying just that. You have been cherry picking your facts, just to make the current admin look bad. I don't have the timwe to list all of the terrorist acts that were committed against us in the last 20 years, but they happen, and inevitably, nobody is held accountable. Get over it.

These guys (the 4 killed in Libya) died for their beliefs, and so that we could maintain our beliefs. But they died because *the current administration* would not send additional security that our Ambassador requested. And the current administration lied to cover their own butts, in front of the bodies of the people they condemned, and in front of the whole country. I would never support Hillary for president.

And there was a drone recording what was happening, phone calls during the attack, and the CIA saying later that day (or the next day) that it was a militant group that attacked, not a demonstration where people happened to bring RPGs and mortars. The government knew what was happening. They chose to lie during the election to make Pres Obama's foreign policy record look better. That's obvious. Again you display an ideological head in the sand over this issue.
